Mastitis: symptoms and VERY important tips to avoid and treat it

Not all unilateral breast redness or pain in a lactating person is mastitis. Important alternative or coexisting diagnoses include:

  • Plugged milk duct (milk bleb) or galactocele.

In uncomplicated mastitis without signs of abscess or severe systemic illness, additional testing is often unnecessary. If symptoms fail to improve after 48–72 hours of appropriate therapy, re-evaluation and ultrasound imaging are recommended.

Management

Antibiotic therapy

Empiric antibiotic selection should cover the usual pathogens while taking into account local patterns of resistance, including community-associated MRSA prevalence.

Common empiric options used in lactation-associated mastitis include:

  • Narrow-spectrum beta-lactamase–resistant agents such as dicloxacillin or nafcillin (when available), or first-generation cephalosporins such as cephalexin, for coverage against methicillin-sensitive Staphylococcus aureus (MSSA) and streptococci.
  • Amoxicillin–clavulanate can be appropriate for broad coverage of common pathogens.
  • If community-acquired MRSA is a concern (or if failure to respond to beta-lactam therapy occurs), agents active against MRSA—such as clindamycin or trimethoprim–sulfamethoxazole (TMP–SMX)—may be selected, mindful of neonatal considerations and local guidance. Typical duration of therapy ranges from 10 to 14 days, depending on clinical response. If there is clinical improvement within 48–72 hours, continuation to complete the course is appropriate. Important considerations during antibiotic therapy:

  • Most antibiotics commonly used for mastitis are compatible with continued breastfeeding. There is generally no need to interrupt breastfeeding when a mother is taking recommended antibiotics; the benefits of continuing to breastfeed typically outweigh potential risks (ACOG; NIH/MedlinePlus).

Abscess management

A breast abscess is a localized collection of pus that may develop when mastitis is not adequately treated or when infection progresses. Clinical features suggesting abscess include a persistent, focal, fluctuant mass, progressive localized pain, and fever despite prior therapy.

  • Ultrasound-guided needle aspiration: For many abscesses, especially those that are small to moderate in size, ultrasound-guided percutaneous needle aspiration (often repeated as needed) is effective and preserves breastfeeding.
  • Incision and drainage: Larger, multiloculated, or refractory abscesses may require surgical incision and drainage under local or general anesthesia.
  • Concurrent antibiotic therapy tailored to culture results is typically given.

Special considerations

Recurrent mastitis

Recurrent or persistent episodes warrant evaluation for underlying causes, such as ductal anomalies, incomplete breast drainage, breast surgery changes, or immune compromise. Imaging and specialist referral (breast surgeon or lactation consultant) may be required.

Nonlactational mastitis

In individuals not breastfeeding, mastitis etiologies differ and may include periductal mastitis or duct ectasia. Smoking is a known risk factor for some nonlactational duct disorders. Management may require different therapeutic approaches and specialist input.

Mastitis and inflammatory breast cancer

Although rare, inflammatory breast cancer can mimic mastitis with diffuse erythema, edema, and rapid progression. Lack of improvement after appropriate antibiotics or atypical clinical features (skin dimpling, peau d’orange, axillary lymphadenopathy in a noninfectious pattern) should raise concern and prompt urgent evaluation, often including imaging and biopsy (ACOG; NIH/MedlinePlus).

Safety of breastfeeding during mastitis and while on antibiotics

In most situations, continued breastfeeding is encouraged during mastitis. Benefits include ongoing drainage of milk, relief of engorgement, and maintenance of supply. When an antibiotic is indicated, most commonly used agents for mastitis are compatible with breastfeeding (e.g., dicloxacillin, cephalexin, amoxicillin–clavulanate, clindamycin). There are very few antibiotics that require breastfeeding interruption; clinicians weigh benefits and risks when prescribing. Practical checklist for mothers and clinicians

  • At first signs of breast pain, localized redness, or lump:
  • Continue breastfeeding on the affected side if tolerable.
  • Apply warm compresses and massage toward the nipple during feeds.
  • Ensure effective latch and try different feeding positions.
  • Increase frequency of emptying (nurse or pump every 2–3 hours).
